Collection of personal information The Company collects the customers' personal information including the data listed in the following items when providing our products and services: (1) Name; (2) Address; (3) Phone number; (4) Email address; (5) Date of birth; (6) Gender; and
(7) In addition to the items listed in the preceding items, data individually notified to customers.

The Company utilizes the customers' personal information listed in each item of the preceding paragraph for the purposes listed in the following items:
(1) For enrolling customers in membership service such as point card service;
(2) For sending customers invitations to sales or events;
(3) For confirming customers' application for e-mail delivery service and delivering e-mail to customers;

Driver's license, health insurance card, basic resident register card with photo, passport, pension book or alien registration certificate. If an agent makes a request for the Disclosure, a power of attorney to indicate the agent's agent is also required.
